RIDE - ALONG PROGRAM
REQUEST & WAIVER

In consideration of the training and/or education I will receive, and the permission granted to accompany members of the Franktown Fire Protection District (the "District") in the course of their duties, I, the undersigned, do hereby release the District from any and all liability, claims, demands, and causes of action which I may hereafter have on account of any and all injuries, damage to my property, or my death, arising out of or related to any occurrence while I am accompanying District personnel on duty.

Incidental thereto, and for the same consideration, I covenant not to sue the aforementioned persons and agree to forever hold them harmless from any such liability or claims. This agreement shall be of full force and effect on the date hereof and on any future occasion when I may accompany any personnel of the District officially.

This release and agreement shall be binding upon me and my heirs, executors, administrators, personal representatives, and assigns, and shall inure to the benefit of the District, its agents, public officials, and successors in office.
